New Patients: What to Expect at Your First Visit

Your first appointment at Greater Austin Pain Center is designed to help our team understand your pain, medical history, previous treatments, and goals for care. During your visit, your provider will review your symptoms, perform an evaluation, discuss any imaging or diagnostic reports you bring, and recommend next steps based on your condition.

Your first visit may include:

  • A review of your medical history and current symptoms
  • Discussion of previous treatments, medications, injections, therapy, or surgeries
  • Review of imaging reports, such as X-rays, MRI, CT scans, or ultrasound results
  • A physical exam focused on your area of pain
  • A personalized treatment recommendation
  • Discussion of whether additional testing, imaging, or procedures may be needed

Please bring your completed patient forms, photo ID, insurance card, medication list, and any medical records related to your condition. If you have not completed your forms before your appointment, please arrive at least 45 minutes early so you have time to fill them out in the office.

Before Your Appointment

Please bring the following items to your first visit:

  • Completed patient forms
  • Photo ID

  • List of current medications
  • Any diagnostic reports or test results from another provider
  • Imaging reports, such as X-rays, MRI, CT scans, or ultrasound results
  • Referral information, if required by your insurance plan
  • Workers’ compensation, personal injury, or accident claim information, if applicable

Bringing these documents helps our pain management team better understand your condition, avoid unnecessary delays, and create a treatment plan based on your medical history and current symptoms.

Transferring Your Care to Greater Austin Pain Center?

Greater Austin Pain Center welcomes patients who are transferring care from another doctor, pain management clinic, specialist, chiropractor, physical therapist, or hospital system. Whether you are looking for a second opinion, continuing treatment after a move, or switching providers, our team can help review your history and determine the next step in your care.

To help make the transition as smooth as possible, please bring or request copies of any records related to your condition, including:

  • Recent office visit notes
  • Imaging reports, such as MRI, X-ray, CT scan, or ultrasound results
  • Procedure notes from prior injections or pain treatments
  • Surgical records, if applicable
  • Physical therapy records
  • Medication history
  • Lab results, if relevant
  • Workers’ compensation, personal injury, or accident-related documentation, if applicable

If another provider has your records, you may need to complete a Release of Medical Records form so those documents can be sent to Greater Austin Pain Center. Having these records available helps our providers avoid unnecessary delays, better understand what has already been tried, and create a treatment plan that fits your current needs.

Even if you do not have all of your records yet, you can still contact our office for guidance. Our team can let you know which documents may be most helpful before your appointment.
